To request transportation service for your child, you must fill out and submit the form below. In addition, you must read and sign the Bus Rider Handbook on the Bus Rider Links tab. Submitting this form indicates that you have read, understand, and agree to comply with the Policies and Procedures contained in the Transportation Bus Rider Handbook.
