REGISTRATION FOR THE CURRENT SCHOOL YEAR(23-24)

Please complete this form and bring your required documents to the student’s campus office to be scanned into your student’s file. 

Required documents:

Birth Certificate of Child(you can order one at www.texas.gov)

Child’s Social Security Card

Current Shot Records

Utility Bill as Proof of Address

Valid ID of Person Enrolling Student- If you are not a parent listed on the birth certificate, you must have a legal document
showing authority to enroll student.

Proof of Income(PRE-K/HEAD START ONLY!)

***All students new and returning must complete forms EVERY YEAR.
